No single policy will fix the fragmentation of our health care system. Rather, a comprehensive approach is required—one that might lead progressively to greater organization and better performance. The following strategies were recommended: (A. Shih, 2008) * Payment reform. Provider payment reform offers the opportunity to stimulate greater organization as well as higher performance. The predominant fee-for-service payment system fuels the fragmentation of our delivery system. We recommend that payers move away from fee-for-service toward bundled payment systems that reward coordinated, high-value care. In addition, we recommend expanding pay-for-performance programs to reward high-quality, patient-centered care. The more organization in delivery systems, the more feasible these payment reforms become (Exhibit ES-1). These payment reforms also could spur organization, since they reward optimal care over the continuum of services. Specifically, we believe that: Patient incentives. Patients should be given incentives to choose to receive care from high-quality, high-value delivery systems. This requires performance measurement systems that adequately distinguish among delivery systems. * Regulatory changes. The regulatory environment should be modified to facilitate clinical integration among providers. * Accreditation. There should be accreditation programs that focus on the six attributes of an ideal delivery system we have identified. Payers and consumers should be encouraged to base decisions on payment and provider networks on such information, in tandem with performance measurement data. * Provider training. Current training programs for physicians and other health professionals do not adequately prepare providers to practice in an organized delivery system or team-based environment. Provider training programs should be required to teach systems-based skills and competencies, including population health, and be encouraged to include clinical training in organized delivery systems. Government infrastructure support. We recognize that in certain regions or for specific populations, formal organized delivery systems may not develop on their own. In such instances, we propose that the government play a greater role in facilitating or establishing the infrastructure for an organized delivery system, for example through assistance in establishing care coordination networks, care management services, after-hours coverage, health information technology, and performance improvement activities. We need to move away from a cottage industry in which providers have no relationship with, or accountability to, one another. Though we acknowledge that creating a more organized delivery system will be difficult, the recommendations put forth in this report offer a concrete approach to stimulate greater organization for higher performance. Shih, 2008) In India, Primary Health Centers (PHCs) are the cornerstone of rural healthcare; a first port of call for the sick and an effective referral system; in addition to being the main focus of social and economic development of the community. It forms the first level of contact and a link between individuals and the national health system; bringing healthcare delivery as close as possible to where people live and work. (Patel, 2005) Primary healthcare services substantially affects the general health of a population, however many factors undermine the quality and efficiency of primary healthcare services in developing countries. In India, although there are many reasons for poor PHC performance, almost all of them stem from weak stewardship of the sector, which produces a poor incentive framework. The World Health Organization (WHO) specifically points out that to some extent, the deterioration in health status is attributed to inadequacies in PHC implementation, neglecting the wider factors that have been responsible for this deterioration such as lack of political commitment, inadequate allocation of financial resources to PHCs and stagnation of inter-sectoral strategies and community participation. The main ones being bureaucratic approach to healthcare provision, lack of accountability and responsiveness to the general public and incongruence between available funding and commitments. The current PHC structure is extremely rigid, making it unable to respond effectively to local realities and needs. For instance, the number of ANMs per PHC is the same throughout the country despite the fact that some states have twice the fertility level of others. Moreover, political interference in the location of health facilities often results in an irrational distribution of PHCs and sub-centers. Government health departments are focused on implementing government norms, paying salaries, ensuring the minimum facilities are available rather than measuring health system performance or health outcomes. Jims intelligence can be seen here, he realizes that nothing good would come from Huck knowing his father was dead in front of him. In Jackson Island Twain gives Jim a sense of compassion, fear and intelligence to make him seem like less of an inferior being to the audience. Twain uses narration to make the audience think of Jim as a human. One example is when Jim and Huck are both in the raft going down the Mississippi river and the fog rolls in. The fog splits Jim and Huck apart from each other, they holler to each other, but end up lost. (63). Jim is suspicious of Huck, Jim knows that Huck is lying to him about something, he just is not sure what it is quite yet. Jim is again shown as intelligent, he is not easily fooled by Hucks pranks like everyone else in the novel. Huck and Jim are able to talk to each other equally, Jim even tells Huck to look him in the eye; most white people would not bother to look a slave in the eye. Twain also shows Hucks view on Jim and his feelings, It was fifteen minutes before I could work myself up to go and humble myself to a nigger- but I done it, and I warnt ever sorry for it afterwards, neither. I didnt do him no more mean tricks, and I wouldnt done that one if Id a knowed it would make him feel that way (65). Here Huck realizes that even though Jim is a slave, he has feelings too. Huck feels so bad for hurting those feelings of Jims that he even manages to apologize to him. Twain also uses Hucks perspective to tell the audience that a young child can think of a slave as a person. Twain challenges people to be like Huck, if a child can treat a slave like a normal person, anyone should be able to do the same. Towards the end of the book, Jim sacrifices his freedom in order to save Tom Sawyer. The doctor that comes to help Tom compliments Jims ability to help with Tom, I had everything I needed, and the boy was doing as well there as he would a done at home better, maybe, because it was so quiet (215). By this point in the story, Twain has drilled the idea into the audience that Jim is an intelligent slave, however, for the first time Twain says so directly through dialogue. The doctor also believes that Jim is a hard worked slave and likes him, Dont be no rougher on him that youre obleeged to, because he aint a bad nigger and yes he was resking his freedom to do it, and was all tired out, too, and I see plain enough hed been worked main hard, lately' (214-215). Jim is characterized to make him seem more human to the audience by making others see what the worth in him is. Jim has taken many actions throughout the novel by this point that exemplify his compassion and his ability to feel emotion, Twain wraps up these points by having a new character give a speech that directly addresses Jim and his characteristics. Twain also uses dialogue from a new character in the story to show that this cannot only be accomplished by children like Huck. Through several different encounters, Jim is characterized through dialogue with others. The character that makes up Jim is his ability to feel emotions and be compassionate while at the same time being intelligent. Through the use of this dialogue, the readers are able to think of Jim as a person and not a slave. At Jackson Island, Cairo and at the Phelps house Jim shows his ability to be intelligent, compassionate and feel emotion. Twain opposes slavery and wants his audience to oppose slavery as well. CEOs participate in formal event; they also make speeches, but in fact have little impact on the daily business operation (Radcliffe, 2010). 3.2 Line leaders are the connections between low and top levels; especially in organizations with high hierarchy They have the power of presenting the executive leaders message to his/her staff. They seek and find opportunities, bring in positive change and make huge efforts towards creating new products/services through new processes and innovative techniques (Kotelnikov, 2001). In addition creative thinkers have the capability to transform ambiguous problems to business opportunities. Besides their ability to analyse the environment, they also take part in building relationships, alliances and encouraging creativity (Fletcher, 2010). Welch also recommends that the manager has to be simple. Simplifying things is a key factor of business (Kotelnikov, 2001). Simple messages travel faster, simpler designs reach the market faster and the elimination of clutter allows faster decision making., according to John Welch Clarifying and sampling things makes it easier for employees to know their position better, as a result they become more energized and focused on achieving objectives. Critical Thinking Critical thinking is the procedure by which an individual comes arises with a new innovative idea (Powell, 2010). The process involves actively and proficiently gathering, analysing and evaluating information, based on an individuals observations, knowledge and past experiences. To be able to think creatively you have to be clear, precise, consistent and fair (The Critical Thinking Community, 2009) Critical thinking could involve integration of ideas which have not been combined before. Brainstorming is one significant form of creative thinking: it works by the unification of two or more peoples ideas in order to create a new innovative one (Powell, 2010). Besides the cooperative efforts taken to produce a new idea, however this process could sometimes be accidental (Brainstorming, 2006). This occurs when a certain circumstance arises and leads someone to start thinking about situations in a different manner, and sometimes leads to a valuable change(Powell, 2010). Nevertheless, new ideas are usually produced through pure use of mutual intelligence and logical sense to come up with something new, this is what could be referred to as a deliberate critical thinking situation. Comparison realizing the similarities and dissimilarities between the ideas you are developing. Also, it includes your ability to make logical associations between the various sources that help you outline and support your ideas. Evaluation Assessing the value of an idea in terms of how is it relevant its to your needs, the facts on which it is based and how it relates to other applicable ideas. Application Transforming the understanding you have gained from the evaluation level in order to use it in response to questions, assignments or projects. Justification Using critical thinking to develop your own arguments, draw conclusions and identify implications (The Open University, 2009). 6. Technology Skills Knowing only how to use basic technology skills such as internet browsing or e-mailing is not enough anymore. Possessing solid technological skills, on the other hand, will put you a step above your colleagues (Pavlina, 2006) In general, companies are extremely reliant on IT that they cant lay off the people who keep their data centre operations running, and neither they are willing let go of their developers, who will be familiar with the forthcoming technology. Financial management is a very important major as I mentioned before, but personally I prefer to be specialised in the accounting finance sector and risks, because I think the accounting field is a sensitive one, also managing risks as well, because it helps ordinary people to understand the market, especially stock exchange market, by using the income and financial statements anyone with a bit knowledge about accounting and finance will be able to figure out the real prices per shares, dividends, return risks and more readings, which will lead us to think more realistic about facts and how to deal with them. Financing risks also very important, it prevents from falling into business traps and circumstances related to it, as well as crisis which can happen anytime and cause big manufactures to collapse, so being aware from risks is a benefit itself; James porter once said avoiding risks is more valuable than the income earned by that risk. There is no legal obligation for companies to draw up management accounts; however, many companies find that it makes running the business much simpler if they do prepare the accounts. In fact, many companies produce account reports monthly, quarterly and yearly to keep track of the financial transaction (Abraham, 2008). However, when utilising an outside agency to launch a product, a company should firstly, have a well-defined product concept, also provide the agency with background information about its products and aims. Technology facilitates the way we live and communicate. Technology, when used efficiently will be a huge assistance for the company. The benefit of technology doesnt only stop at facilitating communication within the organization, but also it has a significant role for new product development. To produce a new product you need to be innovative and you also need the required technology to be available so the process of new product development is formed. And even if sometimes the new product is more expensive, but if it has good quality it can still be sold. Finally, Financial Management and budgeting skills are required by both small firms and giant Multi National Enterprises. Cash flow is the actual capital of the organization. It is important to realize how this capital is built accurately, thats why you have to keep track of all the money you spend in addition to the money you received. Appropriate Financial Management keeps the company away or at least lessen the risks it may face in the future. Looking at previous years trends can sometimes, but not always be useful. Circumstances change regularly, thus a successful manager is one who analyzes the whole environment strategically before taking and business decision. Successfully and efficiently managing is an integrative process. It is true that nobody can be excellent in every skill, being good in some of them, however, and always working on developing the other skills you are not very familiar with will improve your attributes as an individual, an employee or a manager. As a result, possessing business skills and developing them will end you up with a better career and a better life.

Wont that negate freedom if you have to give something up for it? Freedom seems to me to be something that is with out price, unexchangeable for anything ,any reason, any objective. Its irreplaceable in all expressive senses it possesses and represents. It retains and upholds liberty, independence, privilege, and license to be and to live your life as you see suitable. Examples of Informalization in English In linguistics, informalization is the incorporation of aspects of intimate, personal discourse (such as colloquial language) into public forms of spoken and written communication  is called informalization. Its also called demotization. Conversationalization is a key aspect of the more general process of informalization, though the two terms are sometimes treated as synonyms. Some linguists (most notably discourse analyst Norman Fairclough) use the expression border crossing to describe what they perceive as the development in post-industrialized societies of a complex range of new social relationships, with behavior (including linguistic behaviour) . . . changing as a result (Sharon Goodman, Redesigning English, 1996). Informalization is a prime example of this transformation. Fairclough further describes informalization as such: The engineering of informality, friendship, and even intimacy entails a crossing of borders between the public and the private, the commercial and the domestic, which is partly constituted by a simulation of the discursive practices of everyday life, conversational discourse. Multilingual Matters, 1996) Characteristics of Informalization Linguistically, [informalization involves] shortened terms of address, contractions of negatives and auxiliary verbs, the use of active rather than passive sentence constructions, colloquial language and slang. It can also involve the adoption of regional accents (as opposed to say Standard English) or increased amounts of self-disclosure of private feelings in public contexts (e.g. it can be found in talk shows or in the workplace). (Paul Baker and Sibonile Ellece, Key Terms in Discourse Analysis. Continuum, 2011) Informalization and Marketization Is the English language becoming increasingly informal? The argument put forward by some linguists (such as Fairclough) is that the boundaries between language forms traditionally reserved for intimate relationships and those reserved for more formal situations are becoming blurred. . . . In many contexts, . . . the public and professional sphere is said to becoming infused with private discourse. . . . If the processes of informalization and marketization are indeed becoming increasingly widespread, then this implies that there is a requirement for English speakers generally not only to deal with, and respond to, this increasingly marketized and informal English, but also to become involved in the process. For example, people may feel that they need to use English in new ways to sell themselves in order to gain employment. Or they may need to learn new linguistic strategies to keep the jobs they already haveto talk to the public, for instance. In other words, they have to become producers of promotional texts. This can have consequences for the ways in which people see themselves.(Sharon Goodman, Market Forces Speak English. Redesigning English: New Texts, New Identities. Routledge, 1996) The Engineering of Informality in Conversationalization and Personalization [Norman] Fairclough suggests that the engineering of informality (1996) has two overlapping strands: conversationalization and personalization. Conversationalizationas the term impliesinvolves the spread into the public domain of linguistic features generally associated with conversation. It is usually associated with personalization: the construction of a personal relationship between the producers and receivers of public discourse. Fairclough is ambivalent toward informalization. On the positive side, it might be viewed as part of the process of cultural democratization, an opening up of the elite and exclusive traditions of the public domain to discursive practices which we can all attain (1995: 138). To counterbalance this positive reading of informalization, Fairclough points out that the textual manifestation of personality in a public, mass media text must always be artificial. He claims that this sort of synthetic personalization only simulates solidarity, and is a strategy o f containment hiding coercion and manipulation under a veneer of equality. (Michael Pearce, The Routledge Dictionary of English Language Studies. Routledge, 2007) Media Language Informalization and colloquialization have been well documented in the language of the media. In news reportage, for example, the past three decades have seen a definite trend away from the cool distancing of traditional written style and towards a kind of spontaneous directness which (though often contrived) is clearly supposed to inject into journalistic discourse some of the immediacy of oral communication. Such developments have been quantified in textual analysis; for instance, a recent corpus-based study of editorials in the British quality press in the twentieth century (Westin 2002) shows informalization as a trend persisting through the twentieth century, and accelerating towards its end. (Geoffrey Leech, Marianne Hundt, Christian Mair, and Nicholas Smith, Change in Contemporary English: A Grammatical Study. Cambridge University Press, 2010)In an experimental study, Sanders and Redeker (1993) found that readers appreciated news texts with inserted free indirect thoughts as m ore lively and suspenseful than text without such elements, but at the same time evaluated them as less suitable for the news text genre (Sanders and Redeker 1993). . . . But Smith greatly despises the Party and what it stands for, and keeps his feelings about "Big Brother" to himself. One of the turning points in the novel occurs when Smith begins a heated love affair with Julia, a rebellious young girl who works in his agency and shares his dislike for the Party and its beliefs.
